OBJETIVO: Avaliar se a N-Acetilcisteína (NAC) altera o Precondicionamento Isquêmico (PC) em corações isolados de ratos usando apenas um ciclo de PC. MÉTODOS: Freqüência Cardíaca (FC), Fluxo Coronariano (FLC) e Contratilidade Miocárdica (dP/dt) foram registradas em 30 corações de ratos Wistar. Após anestesia, os corações foram perfundidos em sistema de Langendorff com solução de Krebs-Hensleit (K-H), equilibrada (95% de O2 e 5% de CO2). GI: Controle (n=6); GII: 20 min. isquemia (n=6); GIII: PC (n=6); GIV 50 µg/ml/min NAC antes do PC (n =6); GV: 100 µg/ml/min NAC antes do PC (n=6). Todos os parâmetros foram mensurados após 15 minutos de estabilização (T0) e T3, T5, T10, T15, T20, T25 e T30 minutos de reperfusão. Significância estatística foi considerada quando P<0,05. RESULTADOS: Foram observadas alterações na FC comparando GI com GII em T20 e T25 e comparando GI com GIII e GIV com GV em T10 e T20 (P<0,05). FLC foi diferente comparando GI com GII em T3 e T5, GI com GIV em T10 e GI com GV em T10 e T25 (P<0,05). dP/dt foi semelhante comparando GIII com GI e GV. GIII apresentou maior dP/dt que GIV, mas sem diferença estatística (P>0,05). dP/dt foi maior no GV comparado com GIV, mas com diferença estatisticamente significativa somente em T30. CONCLUSÃO: Os corações precondicionados tiveram melhor dP/dt, sendo alteradas pelo uso de NAC no GIV e não alteradas no GV.

OBJECTIVE: The aim of this study is to assess if N-Acetylcysteine (NAC) changes the Ischemic Preconditioning (IP) in isolated rat hearts using only one cycle of IP. METHODS: Heart Rate (HR), Coronary Flow (CF) and Myocardial Contractility (dP/dt) were registered in 30 Wistar rat's hearts. After anesthesia the hearts were removed and perfused with Krebes-Hensleit equilibrated solution with 95% of O2 and 5% of CO2 according Langendorff's method. GI: Control (n=6); GII: 20 min. ischemia (n=6); GIII: IP (n=6); GIV 50 µg/ml/min NAC before IP (n =6); GV: 100 µg/ ml/min NAC before IP (n=6). Parameters were measured after 15 min. of stabilization (T 0) and T3, T5, T10, T15, T20, T25 and T30 min. after reperfusion. Statistical significance was considered when P<0.05. RESULTS: There were changes on HR comparing GI with GII at T20 and T25 and comparing GI with GIII, GIV with GV at T10 and T20 (P<0.05). CF was different comparing GI with GII at T3 and T5, GI with GIV at T10 and GI with GV at T10 and T25 (P<0.05). Myocardial Contractility was similar comparing GIII with GI and GV. GIII had higher dP/dt than GIV but without statistical difference (P>0.05). dP/dt was higher in GV than GIV but with statistically significant difference only at T30. CONCLUSION: dP/dt was better in preconditioned hearts and was changed if using NAC in GIV. The use of NAC didn't change the effects of preconditioning on myocardial contractility in GV.

OBJETIVO: Apresentar a técnica de papilopexia cruzada e seus resultados iniciais na preservação dos músculos papilares em operações de substituição valvar mitral e remodelamento ventricular na insuficiência cardíaca (ICC). MÉTODO: Foram estudados dez pacientes submetidos à cirurgia de troca valvar mitral, sendo sete (70%) do sexo masculino, com idade variável entre 15 e 75 anos (média de 44,4 ± 18,7 anos) portadores de disfunção valvar mitral reumática (50%), prolapso valvar mitral (10%) ou miocardiopatia dilatada e ICC (40%). Após atriotomia e adequada exposição da valva mitral, a cúspide anterior foi desinserida do anel e centralmente dividida, sendo cada metade, com seu complexo de cordas tendíneas, fixada à comissura oposta por sua extremidade medial. Em seguida, foi implantada a prótese valvar mecânica (sete casos) ou bioprótese porcina (três casos) fixada ao anel valvar por meio de pontos separados, com redução do anel valvar mitral, para melhor remodelamento ventricular nos casos com ICC. RESULTADOS: Todos os pacientes receberam alta hospitalar em condições clínicas estáveis, apresentando melhora do desempenho cardíaco no final do primeiro mês de pós-operatório, com redução significativa (p<0,05) do diâmetro sistólico do ventrículo esquerdo e do átrio esquerdo, e fração de ejeção do ventrículo esquerdo aumentando, em média, de 46,7% para 56,4% (p<0,05) no pré e pós-operatório, respectivamente. CONCLUSÃO: A técnica de papilopexia cruzada em operações de tratamento de lesões valvares mitrais e da insuficiência cardíaca permitiu a substituição valvar mitral com recuperação funcional e remodelamento atrial e ventricular favorável e significante.

OBJECTIVE: To present the crossed papillopexy technique and its initial results in the preservation of papillary muscles in mitral valve replacement and ventricular remodeling surgeries for heart failure (CHF). METHOD: Ten patients, 70% male, with ages between 15 and 75 years old (mean 44.4 ± 18.7 years old), suffering from rheumatic mitral valve disease (50%), mitral valve prolapse (10%) or dilated cardiomyopathy (40%), were studied. After opening the left atrium and adequate exposure of the mitral valve, the anterior leaflet already free of its annulus fixation was centrally divided and each half, with its cordae tendineae complex fixed to the commissure on the opposite side. Following this, mechanical (seven cases) or biological (three cases) prostheses were implanted using single sutures in the valve annuli, with reduction of the mitral valve annulus for better ventricular remodeling in CHF cases. RESULTS: All patients were discharged from hospital in good clinical conditions. Additionally all presented with great improvement in the cardiac performance at the end of the first month of follow-up, with significant reductions in the left ventricular and left atrium systolic diameters (p<0.05) and mean increases in the left ventricle ejection fraction of from 46.7 to 56.4 % (p<0.05). CONCLUSION: The crossed papillopexy technique in valve replacement surgeries for mitral valve lesions and CHF presented significant increases in the left ventricular function and improvement of ventricular remodeling in the studied postoperative period.

OBJETIVO: Pesquisando a discinesia de contração miocárdica como outra possível causa da isquemia silenciosa de estresse, analisar a eficácia do cálcio inibidor miocítico diltiazem na normalização de testes ergométricos previamente positivos. MÉTODO: Em outubro de 2004, dez pacientes, sem sintomas de doença arterial coronária, com teste ergométrico positivo definido por infradesnível de segmento ST, sem dor precordial nem arritmia durante os exames (Seis - 60% - do sexo masculino; idades entre 42 e 71 anos, média de 58,2 anos), foram tratados com três doses diárias de 90mg de diltiazem e reestudados após período de cinco dias. Os exames foram realizados em esteira ergométrica, no protocolo de Bruce. RESULTADOS: O diltiazem impediu a depressão do segmento ST, tanto do ponto J (infradesnível médio e 2,1 ± 0,3mm, no controle, e 0,0 desnível com tratamento; p<0,001), quanto do ponto Y (infradesnível de 1,65 ± 0,7 mm, no controle, e 0,0 desnível com tratamento; p<0,001). A freqüência cardíaca não apresentou variação estatisticamente significante (média de 156,1 ±12,3 bpm, no controle, e 151,6 ± 23,4 bpm com tratamento/ p>0,05). CONCLUSÃO: A administração do cálcio inibidor miocítico, diltiazem, impediu a depressão silenciosa do segmento ST previamente induzida por teste ergométrico em pacientes sem sintomas de doença arterial coronária, confirmando o envolvimento da discinesia da contração miocárdica na gênese do fenômeno.

OBJECTIVE: To research the myocardium stress diskinesia as another possible cause of the silent myocardium ischemia induced by exercise testing stress and to analyze the efficacy of the myocytic calcium blocker diltiazem for normalization of previously positive testing. METHOD: In October 2004, ten patients without symptoms of coronary artery disease, presenting positive exercise testing, defined by ST segment depression, with neither precordial pain or arrhythmia during testing (six males - 60%; ages between 42 and 71 years old, mean 58.2 years), were treated with 90 mg of diltiazem in three times daily and re-studied five days after the first examination. Treadmill electrocardiography exercise testing was performed using the Bruce protocol. RESULTS: Diltiazem administration blocked the ST segment depression, both J point (mean 2.1 ± 0.3mm, in the control and 0.0 depression with treatment; p<0.001) and Y point (mean 1.65 ± 0.7 mm in the control and 0.0 depression with treatment; p<0.001). The Heart rate variations were not significant (p>0.05), with mean values of 156.1 ±12.3 in the control and 151.6 ±23.4 bpm with treatment). CONCLUSION: The administration of the myocytic calcium inhibitor diltiazem impeded the occurrence of the silent ST segment depression, previously induced by exercise stress testing in patients without symptoms of coronary arterial disease, confirming the involvement of the myocardium contraction diskinesia in the phenomenon genesis.

A cirurgia do "Labirinto" para tratamento da fibrilação atrial primária, inicialmente proposta por Cox, é relativamente complexa e aumenta o risco de morbidade próprio da cirurgia da valva mitral isoladamente. OBJETIVO: Apresentar o detalhamento técnico, os conceitos de eletrofisiologia adotados e os resultados iniciais de nova abordagem cirúrgica e eletrofisiológica, incluindo o bloqueio de macrocircuitos atriais específicos definidos por Frame, e otimizando a tática cirúrgica de tratamento da fibrilação atrial. MÉTODO: Foram operados oito pacientes portadores de fibrilação atrial crônica e disfunção de valva mitral, associada à disfunção de valva tricúspide em um paciente, empregando-se as seguintes modificações principais da cirurgia de Cox: 1- Exclusão da aurícula esquerda por sutura interna, na junção com o átrio esquerdo; 2- Exclusão de aurícula direita por sutura em bolsa de fixação da cânula de drenagem venosa para a veia cava superior; 3- Incisão biatrial única; 4- Eletrocauterização transendocárdica no tecido atrial esquerdo circunjacente aos óstios das veias pulmonares; 5- Substituição das incisões e suturas no átrio esquerdo por eletrocauterização transendocárdica. RESULTADOS: O tempo de circulação extracorpórea variou de 64 a 133 min (média de 107,5 min) e o tempo de cardioplegia de 40 a 105 min (média de 76,7 min). Ao final da cirurgia, todos os pacientes estavam em ritmo atrial regular. O pós-operatório transcorreu sem complicações cirúrgicas e todos os pacientes receberam alta hospitalar em ritmo atrial regular. Em nenhum paciente foi necessário o implante de marcapasso definitivo. No seguimento ambulatorial, seis meses após a cirurgia, seis (75%) pacientes mantiveram o ritmo atrial com as contrações atriais regulares, e dois apresentaram fibrilação atrial, clinicamente controlada (NYHA II). Também não ocorreram complicações embólicas nem evidência de trombose no controle ecocardiográfico. CONCLUSÃO: Pode-se concluir, como experiência inicial, que a abordagem eletrofisiológica e a técnica empregada otimizaram o tratamento cirúrgico da fibrilação atrial, possibilitando a correção de lesões valvares mitrais e tricúspide sem morbidade adicional.

INTRODUCTION: The maze procedure for primary atrial fibrillation treatment, initially proposed by Cox, is somewhat complex and increases the morbidity risk originating from the isolated mitral valve surgery itself. OBJECTIVE: To describe the surgical technique, the concepts of electrophysiology considered and the initial results of a new surgical and electrophysiological approach. Also the blocking of main atrial circuits as defined by Frame and optimization of the surgical tactic for atrial fibrillation treatment are considered. METHOD: Eight patients with chronic atrial fibrillation and mitral valve dysfunction and with tricuspid valve regurgitation in one case were operated on. The following modifications of the classic Cox procedure were employed: 1. - Exclusion of the left atrium appendage by internal suturing to close its left atrial ostium; 2. - Exclusion of the right atrium appendage by one purse string suture used for the fixation of the superior vena cava draining cannula; 3. - Single atrial incision; 4. - Transendocardial electrocauterization of the left atrium wall around all the pulmonary vein ostia; 5. - Substitution of the incisions and sutures in the left atrium by transendocardial electrocauterization. RESULTS: The cardiopulmonary bypass time varied from 64 to 133 min (mean 107.5 min) and the cardioplegia time was from 40 to 105 min. (mean 76.7 min). At the end of the surgery all patients were in regular atrial rhythm. The postoperative period was uneventful with all patients discharged from hospital showing regular atrial rhythm, without definitive pacemaker implantation. Six months after surgery, six patients were in regular atrial rhythm (75%) with preserved atrial contractions, and two (25%) with atrial fibrillation, clinically controlled (NYHA II). There was no evidence of embolic complications or thrombosis seen by Doppler echocardiography. CONCLUSION: It is concluded that in this initial series of cases, the electrophysiolgical approach and the surgical technique employed improved the surgical treatment of atrial fibrillation enabling the correction of mitral and tricuspid valve lesions without additional morbidity.

FUNDAMENTOS: As varizes de membros inferiores constituem uma das mais comuns doenças circulatórias, contudo, até onde foi possível alcançar, não foram encontradas na literatura médica publicações científicas descrevendo a incidência comparativa de regurgitação valvar em veias ilíacas e safenas magnas. OBJETIVO: Estudar a ocorrência de regurgitação valvar em veias ilíacas, associada ou não com insuficiência de válvulas em veias femoral ou safena magna. MÉTODO: Em 30 pacientes portadores de varizes em membros inferiores, com idade entre 21 e 71 anos (média de 39 anos), sendo seis (20 %) do sexo masculino, foram estudados exames Duplex Scanning (Aparelho Esaote; transdutor de 7,5 MHZ, com técnica bidimensional, doppler pulsado e fluxo a cores), analisando a regurgitação em veias ilíaca, femorais , safenas magnas e poplíteas. RESULTADOS: A regurgitação ilíaca importante, igual ou superior a 26,0 cm/s presente em 19 membros, ocorreu isoladamente, sem refluxo na válvula ostial da veia safena em apenas um (5,3%) caso. Nos casos com regurgitação ilíaca menor (Classes 1 a 3), a ausência de regurgitação ostial da veia safena ocorreu em quatro (14,3%) membros. No total de 60 membros estudados, houve incidência de regurgitação ilíaca em 47 (78,3%) casos, e de disfunção valvar ostial da safena magna em 45 (75%) casos. Contrariamente, a regurgitação ostial safena sem refluxo ilíaco ocorreu somente em três (6,3%) casos, sendo em apenas um caso (2,1%) superior a 25,9 cm/s. CONCLUSÃO: Concluiu-se que as anomalias da função das válvulas em veias ilíacas representam o fator determinante de maior correlação com a intensidade da insuficiência valvular ostial da veia safena magna e com a gravidade das varizes em membros inferiores.

BACKGROUND: Varicose veins of the lower extremities is one of the most common circulatory diseases but no published reports describing the associated comparative rates of femoral and iliac valve regurgitation were found. OBJECTIVE: To study the occurrence of selective valve regurgitation in the external iliac vein, associated or not with insufficiency of the valves in the femoral or great saphenous veins. METHOD: With the approval of the Ethics Committee of the São Francisco de Assis Cardiovascular Foundation, the venous system of the lower limbs of twenty-four female (80%) and six male (20%) patients aged between 21 and 71 years old (mean 39 years old), with varicose veins in the lower extremities, were studied with Color Flow Duplex Scanning (Esaote array; 7.5 MHz transducer; 2-dimensional, pulsed Doppler and color flow techniques). The aim was to investigate the regurgitation in the iliac, femoral, popliteal and great saphenous veins. The results were statistically analyzed using variance analysis and student T-test with a significance level of 0.05. RESULTS: Iliac regurgitation ³ 26.0 cm/s, which was present in 19 limbs, occurred isolated without reflux in the great saphenous ostium valve only in one case (5.3%). In the cases with iliac regurgitation < 26.0 cm/s, the absence of regurgitation in the great saphenous vein ostium occurred in four limbs (14.3%). Considering the total of 60 lower limbs studied, iliac regurgitation was observed in 47 (78.3%) cases and dysfunction of the saphenous ostium valve in 45 (75%) cases. Furthermore, saphenous ostium valve reflux without iliac regurgitation occurred only in three cases (6.3%), which in only one case was above 25.9 cm/s. CONCLUSION: In conclusion anomalies of the valvular function in the iliac veins present a significant correlation with the intensity of the insufficiency in the great saphenous vein ostium valve and the gravity of varicose veins in the lower limbs.

OBJETIVO: Apresentação de modelo aperfeiçoado de prótese intraluminal sem sutura para substituição segmentar da aorta. MÉTODO: Empregou-se anel de plástico, sulcado, multiperfurado, facilitando a fixação e confecção intra-operatória da prótese vascular. Um paciente de 42 anos, do sexo masculino, portador de aneurisma dissecante da aorta, tipo A, foi operado com técnica de Bental - De Bono, empregando-se tubo valvulado de pericárdio bovino com implante proximal por meio de pontos separados e fixação distal sem sutura. RESULTADOS: O modelo aperfeiçoado de anel facilitou a confecção e fixação da prótese vascular na aorta, distalmente, sem sutura. O paciente operado evolui assintomático um ano após a operação. CONCLUSÃO: A prótese intraluminal aperfeiçoada facilitou a cirurgia de substituição segmentar da aorta.

OBJECTIVE: To report on an improved model of intraluminal sutureless prosthesis for partial aortic replacement. METHOD: A grooved multidrilled plastic ring was employed to facilitate the intraoperative handcrafting of the vascular prosthesis and its sutureless intraluminal fixation to the aorta. A 42 years old male patient with type A Dissecting Aneurysm was treated by the Bentall - De Bono technique using a valved and ringed bovine vascular prosthesis which was implanted in the aortic annulus with interrupted sutures and tied distally inside the aorta. RESULTS: The improved ringed vascular prosthesis simplified the sutureless intraluminal anastomosis procedure. The postoperative evolution was uneventfull and the patient remains asymptomatic one year after the operation. CONCLUSION: The improved ringed vascular prosthesis simplified the intraluminal sutureless procedure for treatment of dissecting aneurysm of the aorta.

Foram estudados, pela técnica de Langendorff, 18 corações isolados de ratos albinos adultos, submetidos a perfusão retrógrada pela solução de Krebs-Henseleit (pressão de 90 ± 10 cm de H2O, temperatura de 36,5 ± 0,5°C). Os animais foram distribuídos nos seguintes grupos: Grupo I: controle; Grupo II: tratado com isoflurano na concentração de 1,5%. Foram avaliados os seguintes parâmetros: freqüência cardíaca (FC), pressão sistólica (PS), pressão diastólica (PD), dp/dt e fluxo coronariano (FCo). Após 15 min de estabilização, foram obtidos os valores iniciais (t0), sendo os parâmetros registrados seqüencialmente nos seguintes períodos: 1, 2, 4, 6, 10 min. Não ocorreram variações estatisticamente significantes na PS nem na dp no período t1 nos dois grupos estudados. Notou-se aumento da PD (p < 0,05) apenas no Grupo II (4,8 mmHg no período t0, 12,8 mmHg no período t1). Ocorreram ainda diminuição da FC (p < 0,05) e aumento do FCo (p < 0,05) um minuto após a infusão da droga. Notou-se diminuição do FCo de forma progressiva em todos os períodos de recuperação (t2 a t5) após a infusão do isoflurano (p < 0,05). Conclui-se que o isoflurano aumenta o fluxo coronariano durante a sua administração, reduzindo-o no período de recuperação; diminui a freqüência cardíaca e aumenta a pressão diastólica em corações isolados de ratos.

Eighteen adult rat hearts were studied by Langendorff's Technique under Krebs-Henseleit solution perfusion (90 ± 10 cm of H2O pressure, 36.5 ± 0.5°C temperature). Nine rats were assigned to two groups: Group I - control group and Group II - isoflurane group. Hearts in Group II were treated with isoflurane at 1.5%. After stabilization during 15 minutes, initial values were obtained (t0), the next parameters were registered over the following periods: 1, 2, 4, 6, 10 minutes, respectively. There were no statistically significant variations in systolic pressure nor in dp/dt (p > 0.05) in period t1 in both groups. An increase was observed in diastolic pressure, statistically significant in Group II (4.8 mmHg - t0 period; 12.8 mmHg - t1 period). There was a decrease in heart rate and increase in coronary flow in Group II, statistically significant(p < 0.05), one minute after drug infusion. The coronary flow decreased progressively in every recovery period (t2 t0 t5) in Group II (p < 0.05). During the same periods there were no statistically significant variations in Group I. Isoflurane increases coronary flow during its administration, decreasing it during the recovery periods; it decreases heart rate, increases diastolic pressure and shows evidence of myocardial contractility depression in isolated rat hearts.

É apresentado aperfeiçoamento em técnica de cardioplegia retrógrada atrial, consistindo na passagem de fita cardíaca pelo seio transverso do pericárdio. As extremidades da fita passando pelo interior do tubo de látex formam torniquete que permite a oclusão do tronco pulmonar sem necessidade de sua dissecção na contigüidade com a aorta ascendente. Estando as veias cavas ocluídas sobre as cânulas de circulação extracorpórea, perfunde-se continuamente cardioplegia sangüínea (25 mEq/l de K) hipotérmica (4 - 8°C) por cateter introduzido no átrio direito; os óstios coronarianos são diretamente perfundidos com intervalos de 20/30 min. Foram operados 5 pacientes submetidos a procedimentos na aorta ascendente (operação tipo Bental - De Bonno, 1 caso; substituição de valva aórtica, 3 casos; substituição de valva aórtica associada a correção de aneurismas de artéria coronária direita e aorta ascendente, 1 caso), com tempo de pinçamento aórtico variável entre 80 e 180 min (média de 107,8 min). Todos os pacientes saíram de circulação extracorpórea com autonomia hemodinâmica. O pós-operatório imediato e hospitalar foi normal. Todos os pacientes receberam alta em boas condições clínicas.

An improved technique of atrial, retrograde cardioplegy is presented where the pulmonary artery trunk and the ascending aorta were snared with a tape passing behind both vessels through the transverse sinus of the pericardium. The pulmonary artery occlusion was controlled by the tourniquet avoiding its surgical isolation from the ascending aorta. With both venae cavae snared, blood / potassium (25 mEq/l) continuous cardioplegy was delivered through a catheter inserted in the right atrium ; the coronary ostia were directly perfused at 20 / 30 min intervals. Five patients were submitted to surgical procedures in the aortic root (Bental - De Bonno surgery, 1 case; aortic valve replacement, 3 cases; aortic valve replacement associated with surgical correction of aneurysm in the right coronary artery and ascending aorta, 1 case. The aorta crossclamp time ranged from 80 - 180 min. (mean 107.8 min.). All patients weaned from by-pass with haemodynamic autonomy. Immediate and in hospital post-operative recovery was normal . All patients were discharged in good clinical conditions.

Estudou-se o padrão de distribuição miocárdica de solução cardioplégica (SC) cristalóide gelada (3ºC - 4ºC) perlundida nos corações de 15 cães mestiços com pesos variáveis entre 10-15 kg. Após anestesia e toracotomia mediana anterior, o pericárdio foi aberto, sendo estabelecida circulação extracorpórea. As seguintes vias foram empregadas para injeção cardioplégica: 1) Anterógrada - por canulação da aorta ascendente a montante da pinça de oclusão; 2) Retrógrada Seletiva - através de cânula com balão auto-inflável introduzido no seio coronário (SCo); 3) Retrógrada Total - através de cânula introduzida no átrio direito (AD); 4) Retrógrada Seqüencial SCo-AD - com a SC injetada primeiro pelo seio coronário até a temperatura dosepto interventricular atingir 16ºC e, em seqüência, pela cãnuia no átrio direito como na técnica retrógrada total, como tronco arterial pulmonar ocluído;5)Retrógrada Seqüencial SCo-VD-com a cavidade do ventrículo direito perfundida por cânula passada através da valva tricúspide. Controlou-se a variação da temperatura miocárdica no ventrículo esquerdo, VD, AD e região dono sinoatrial, por meio de teletermòmetro Ômega com termistor de agulha. Pode-se constatar que o esfriamento cardíaco uniforme, o menor volume e o menor tempo de injeção ocorreram com a técnica anterógrada, seguida em excelência pelas técnicas retrógradas seqüenciais SCo-AD e SCo-VD. Concluiu-se que a técnica de cardioplegia retrógrada sequencial é significantemente melhor que as retrógradas seletivas pelo SCo e total pelo AD, como usualmente empregadas para proteção miocárdica, quando comparadas com a técnica de perfusão anterógrada pela aorta.

The distribution pattern of a cold (3-4ºC), crystalloid cardioplegic solution (CS) in the myocardium was studied in 15 mongrel dogs, with 10-15 kg of body weight. Alter anesthesia and median sternal thoracotomy. The pericardium was opened and extracorporeal circulation established. The following routes were employed for cardioplegic perfusion: 1) Antegrade - through ascending aortic canullation bellow the aortic occlusion clamp;2) Selective retrograde - through coronary synus (Co.S - 25 mmHg) using a self-inflating ballooned cannula; 3) Total retrograde, (Co.S - 40 mmHg) - through a cannula inserted in the right atrium (RA); 4) Sequencial retrograde, Co.S-RA - with the CS flowing first through the coronary synus lowering the interventricular septal temperature to 16 ºC and after through the RA cannula as in the total retrograde technique with the pulmonary artery occluded and;5) Sequencial retrograde, Co.S-RV - the RV chamber being directly cannulated through the tricuspid valve and perfused, instead of the RA in the latter technique. The temperature variation of the myocardium in the left ventricule (LV), RVt RA and sinus node region (SN) was controlled employing an Omega needle termistor and thermometer. With the antegrade technique (70 mmHg pressure) the most uniform myocardial cooling, the lowest CS volume and perfusion time duration was observed, followed In excelence by the Co.S-RA Sequencial retrograde technique and the Co.S-RV sequencial technique. The present data indicate that sequencial retrograde cardioplegic perfusion techinique is significantly better than the usual Co.S or RA total retrograde technique alone for myocardial protection when compared with the aortic root antegrade perfusion technique.
